St. Peter in Chains Registration Information

You may print the applicable registration PDF file and complete the necessary forms by hand or use your computer to fill-in the information then print the completed forms.   Both methods require the freely available Adobe Reader software installed on your computer.  If you have questions or need assistance, please contact Mrs. Jacobs (jacobst@stpeterinchains.org or  call 863-0685 ext 306). 

To use your computer to fill-in the information:

For Adobe Reader versions 5.5 and above, after opening the document click Highlight Fields located at the top right side of the document.  This will change the appearance of the items where information should be entered.  For all versions, input the appropriate data in each field by using the Tab key to move from one field to the next.  Also, you can use your cursor to move from field to field.
