2012-2013 Registration Information Thank you to all parents who were present Sunday, January 29 at our Open House. If you wish to fill out registration papers please click on the link below. To register and be eligible for the discounted registration fee please return the student record form and paid fee on or before Friday February 3, 3:00 pm. The following items may be returned at a later date: Student Sacrament Form, Health/History Form, Medical Record, Dental Record, Baptism and Birth Certificates. Completed registration forms and paid fees that are received during Catholic Schools Week will be eligible for a discounted registraton fee. Early registration discounted applicable through 02/03/12 $ 50.00/student $ 100.00/2 or more students Registration fee after 02/03/12 $ 75.00/student $ 150.00/2 or more students Registration Packets To all new students enrolling for the 2012-2013 school year, the Student Record form and paid registration fee may be returned without the following forms: Student Sacrament Form, Health/History Form, Medical Record, Dental Record, Baptism and Birth Certificates.
